REVELATION DIAGNOSTICS PARTNER PROGRAM PRIVACY POLICY

This Privacy Policy explains how Revelation Diagnostics, LLC (“Revelation Diagnostics,” “RD,” “we,” “us,” or “our”) collects, uses, shares, and protects personal information when Partners (“you” or “your”) enroll in or participate in the Revelation Diagnostics Partner Program (the “Program”).
This Privacy Policy applies only to individuals applying for or participating in the Program (including licensed clinicians, unlicensed health coaches, and other professionals).
It does not apply to patient data collected through lab testing services; such data is handled under our Consumer Privacy Policy and HIPAA Program / Business Associate Agreements, where applicable.
By using the Program or submitting personal information, you consent to the practices described in this Privacy Policy.
- Information We Collect About Partners
We collect the following categories of information to operate the Partner Program:
A. Information You Provide Directly
- Name
- Email address
- Phone number
- Mailing or business address
- Professional details (e.g., license number, NPI, credentials—if applicable)
- EIN or SSN (only if required for tax reporting)
- Payment and tax information (e.g., W-9, PayPal/Stripe details)
- Website(s), social media handles, or promotional channels
- Information submitted during interpretation work (non-PHI unless authorized)
- Communications with RD
B. Automatically Collected Information
When you access the RD partner portal (including via GoAffPro):
- IP address
- Browser and device information
- Access logs
- Referral link activity
- Clickstream and analytics data
- Cookies (see Section 7)
C. Information From Other Sources
We may receive additional information about you from:
- Public records or professional licensing databases
- Third-party verification services
- Payment processors
- Referral partners
- Customers who identify you as their referring Partner
- Social media accounts (if you interact with RD through them)
D. Patient Information
Partners may only access PHI under the following conditions:
- The patient expressly authorizes RD to share their information with you; OR
- You have a valid Business Associate Agreement (BAA) with RD; OR
- You access only non-PHI interpretation summaries (for unlicensed Partners)
Unauthorized access to PHI is strictly prohibited.
- How We Use Partner Information
We use your information for the following purposes:
A. Program Operation
- Verifying identity and eligibility
- Managing your Partner account
- Tracking referrals and interpretation activity
- Administering Partner Fees and payments
- Maintaining tax and accounting records
- Providing support, updates, and program communications
B. Compliance & Licensing
- Verifying professional credentials (if applicable)
- Ensuring compliance with healthcare advertising + scope-of-practice laws
- Fulfilling HIPAA obligations (for licensed clinician Partners)
C. Platform Functionality
- Operating the RD partner portal and analytics
- Improving features, security, and tracking
- Detecting fraud or misuse
D. Marketing & Program Development
- Sending updates about Program changes, benefits, and opportunities
- Creating anonymized or aggregate reports
- Communicating about new RD services, products, and offers

E. Legal and Regulatory Uses
- Meeting tax, audit, or regulatory obligations
- Responding to lawful requests, subpoenas, or regulatory inquiries
- Protecting the safety, rights, or security of RD or its users
- We do not sell Partner personal information.
- How We Share Partner Information
We share information only as necessary to operate the Program:
A. Service Providers
Including:
- Payment processors
- IT and hosting providers
- Identity verification services
- Email and communication platforms
- Analytics partners
They may access personal information only to perform services on our behalf.
B. Customers
We may share your:
- Name, credentials, profile, and role
- Interpretation contributions (with customer authorization)
- PHI is shared only as permitted by law and applicable BAAs.
C. Legal and Regulatory Authorities
We may disclose information when required to:
- Comply with law or legal process
- Enforce our agreements
- Respond to audits
- Prevent fraud or harm
D. Business Transfers
If RD is involved in a merger, acquisition, financing, or sale of assets, Partner information may be transferred consistent with this Policy.
We do not disclose Partner information for third-party advertising or sell your information.
- Partner Access, Choices, and Account Management
You may:
- Access or update your Partner profile
- Change payment or tax information
- Update contact information
- Opt out of marketing communications
- Request deletion of your Partner account (subject to legal retention requirements)
- Some information, such as financial/tax records or fraud logs, cannot be deleted while legally required to be retained.
- Data Security
We use administrative, technical, and physical safeguards to protect personal information, including:
- Encrypted data transmission (SSL/TLS)
- Access controls and credential management
- Audit logging
- Regular system monitoring
- Vendor security review
- However, no system is 100% secure, and Partners must protect their login credentials.
Partners accessing PHI must comply with HIPAA security requirements.
- Data Retention
We retain Partner information as long as:
- You maintain a Partner account
- Required for tax, legal, or regulatory purposes
- Necessary to resolve disputes or enforce agreements
Interpretation contributions may remain anonymized for RD product improvement.
- Cookies, Tracking, and Analytics
RD’s partner portal (including through GoAffPro) uses:
- Cookies
- Analytics scripts
- Tracking pixels
- Referral link tracking IDs
You may disable cookies, but certain Partner Program features may not function.
We do not track users across third-party websites for behavioral advertising.
- Children’s Privacy
The Partner Program is not intended for anyone under 18 years of age.
We do not knowingly collect personal information from minors.
- International Transfers
If you access our systems from outside the United States, you acknowledge that your information will be processed in the U.S. under U.S. privacy laws.
- Changes to This Privacy Policy
We may update this Privacy Policy periodically.
If changes materially affect your rights, we will notify you through:
- Email,
- The Partner Portal, or
- The Program signup page.
Your continued participation in the Program constitutes acceptance of any changes.
